The dates the work should begin and end
Contact information for responsible parties

Choosing your route
When two state routes run common, it is customary to use the lower route number. Example: SR 8 and SR 5, the SR 5 number will be used. The exception will be when a lower numbered route is common with either an Interstate Route or a 500 Series Route i.e.(SR 520,515) Additionally, when a lower numbered route that has a suffix (business; connectors; loops; spurs; etc.) example: SR 15 and SR 4BU the SR 15 number will be used disregarding the alternate route number.
Permits for GDOT Work
If your permit is in conjunction with an existing GDOT project, you are off to a head start. You circumvent the location identification process since GDOT has already determined the location. You will not have to upload drawing files or determine whether blasting will occur, since GDOT has already determined those aspects. Use the Apply by Project ID tab to complete this type of permit request, and enter the project ID. (See image below.)
